Insurance Requirements for UTC Permits
All companies that apply for a UTC permit, certificate, or registration must have proof on file that they have public liability and property damage insurance. Household goods companies also need cargo insurance. This proof of insurance must also stay up to date.
Proof of Insurance
Form E
Form E is the standard proof of insurance form. It is issued by your insurance company, and its full name is the Uniform Motor Carrier Bodily Injury and Property Damage Liability Certificate of Insurance.
Certificate of Insurance or Binder
A certificate or binder can be used for up to 60 days. After that, it must be replaced by Form E.
These documents must include:
- The UTC listed as the certificate holder
- The amounts of insurance coverage
- The effective dates of coverage
- A valid signature

General commodities
- For vehicles under 10,000 pounds: $300,000 Combined Single Limit (CSL) minimum.
- For vehicles over 10,000 pounds: $750,000 Combined Single Limit (CSL) minimum.
Hazardous substances, any weight
- For bulk hazardous substances, explosives, poison gas, compressed gas, and radioactive materials: $5 million Combined Single Limit (CSL) minimum.
- For other hazardous waste or materials: $1 million Combined Single Limit (CSL) minimum.
Household Goods Carriers
Bodily Injury and Property Damage Liability
- For vehicles under 10,000 pounds: $300,000 Combined Single Limit (CSL) minimum.
- For vehicles over 10,000 pounds: $750,000 Combined Single limit (CSL) minimum.
Cargo
- For vehicles under 10,000 pounds: $10,000 minimum.
- For vehicles over 10,000 pounds: $20,000 minimum.
Solid Waste Collection Companies
