On March 30, 2015, the commission issued a 
penalty assessment against Tom Wise, Tank Wise LLC (Tank Wise or Company) in the amount of $2,000, alleging 2 violations of RCW 19.122.030(1)(a) by failing to request a dig ticket prior to performing an excavation. Tank Wise filed with the commission a request for hearing on April 9, 2015.
On April 24, 2015, the commission issued a notice of brief adjudicative proceeding, which scheduled oral statements in this matter for Friday, June 5, 2015.
On June 15, 2015, the commission entered Order 01 - Initial Order; Affirming Penaly Assessment.
