On February 19, 2015, the commission filed a complaint against Qwest Corporation d/b/a CenturyLink QC (CenturyLink) for violations of state laws and Commission rules arising from a 911 service outage affecting Washington residents on April 9-10, 2014. Commission staff has requested an order assessing monetary penalties and requiring CenturyLink to implement certain technical and procedural safeguards designed to prevent reoccurrence of the outage. .
On March 20, 2015, the commission issued a notice setting the prehearing conference for April 20, 2015, at 9:30 a.m., at UTC headquarters in Olympia, Washington.
On April 22, 2015, the commission entered Order 01, Prehearing Conference Order, setting a procedural schedule in this matter.
On August 6, 2015, Staff filed with the commission, on behalf of the parties, a request to modify the procedural schedule. By notice, issued August 6, the commission revised the procedural schedule.
On September 10, 2015, Staff and CenturyLink filed with the commission a Multiparty Settlement Agreement that would resolve all issues in this matter.
On September 14, 2015, the commission revised the procedural schedule and issued a notice of hearing on the settlement proposal.
The commission held a settlement hearing on the proposed settlement agreement on January 12, 2016, beginning at 9:30 a.m., at UTC headquarters, Olympia, Washington.
On February 22, 2016, the commission entered Order 03, Final Order Approving Settlement Agreement.
